School Entry Requirements
Sports Physical: Any student who will be in grades 7-12 for the 2017-18 school year, and wishes to participate in extra curricular athletics must have a current sports physical done after May 1, 2017 as directed by the Kansas State High School Activities Association.
Immunization record: Please bring a copy of your child's immunization record with you when enrolling, if you are new to the district or have had any updates.
Diphtheria, Tetanus, Pertussis (DTaP): Five doses required. Four doses acceptable if dose 4 given on or after the 4th birthday. A single dose of required at Grades 7-10 if no previous history of Tdap vaccination regardless of interval since the last Td.
Poliomyelitis (IPV/OPV): Four doses required. Three doses acceptable. One dose required after age 4 regardless of the number of previous doses, with a 6 month minimum interval from the previous dose.
Measles, Mumps, Rubella (MMR): Two doses required.
Hepatitis B: Three doses required through grade 12.
Varicella (chickenpox): Two doses required for grades K-12 or history of varicella disease documented by physician.
Administer Medicine: All medicines distributed at an Abilene School must have a Request to Administer Medicine form on file.
School Entry Physicial: All students between the ages 5-8 who is enrolling in a Kansas school for the first time must also present a completed physical examination that has been done in the previous 12 month period.
Food Allergy/ Modification: Any child that has a food allergy/intolerance or other dietary needs, will need to have a dietary form on file. This form needs to be filled outeach year and signed by the child's physician.
