Sports Physical: New form update on May 1, 2025. Any student who will be in grades 7-12 for the 2025-2026 school year, and wishes to participate in extra curricular athletics must have a current sports physical done after May 1, 2025 as directed by the Kansas State High School Activities Association. Sports Physical night will be held on May 14, 2025 at the Memorial Health System in Abilene.

Immunization record: Please bring a copy of your child’s immunization record with you when enrolling if your student is new to the district or has had any updates. (Spanish Form Version)

Medication: All medication administered at school must have a “Request to Administer Medication” form completed and signed by your physician and on file with the nurse.

School Entry Physical: Students (ages 4 - 8)  who are enrolling in a Kansas school for the first time must have a completed physical examination that has been done in the previous 12 month period.  

Food Allergy/ Modification: Any child that has a food allergy/intolerance or other dietary needs, is required to have a dietary form on file with food service.  This form needs to be filled out each year and signed by the child's physician or dietician.
